Is there a way to tell which master rewards you've received for which quests rather than going back and redoing all of them for the Protector title? Thanks!

You can tell by looking at the icon displayed on the mission location from map view. In proph its displays the helm with both swords. In nightfall you'll see it with 3 spears. In factions the same manner (sorry don't recall the exact weps...daggers may be.).

If you didn't achieve masters one of the weps would be missing.

This is correct; though both Factions and Nightfall mission icons have two swords and a spear to signify completion level (Proph is two swords as stated above).
